The short version

Creatine has spent forty years as a strength supplement, and the last few as a cognitive one. The interesting thing about the second career is that the evidence is not weak — it is conditional. It shows up in specific people under specific strain and it disappears in everyone else, and once you see which people, the whole literature makes sense.
The meta-analysis, split by age
A 2023 review in Nutrition Reviews found ten randomised trials of creatine and memory in healthy people and pooled eight of them. Overall, creatine improved memory measures against placebo: SMD 0.29 (95% CI 0.04 to 0.53, P = 0.02).
Then the subgroup analysis. In older adults aged 66 to 76, the effect was SMD 0.88 (95% CI 0.22 to 1.55). In participants aged 11 to 31, it was 0.03 (95% CI -0.14 to 0.20, P = 0.72) — which is a statistical way of saying nothing happened. Dose ranged from 2.2 to 20g a day and duration from five days to 24 weeks, and neither predicted the result. Age did.
An earlier systematic review of six trials in 281 people reached the same shape from a different angle: some evidence for short-term memory and reasoning, conflicting results elsewhere, and performance unchanged in young individuals. It also noted that vegetarians responded better than meat-eaters on memory tasks.
The vegetarian finding, which is the tell
That vegetarian result is worth its own paragraph, because it is the cleanest natural experiment available. Dietary creatine comes from meat and fish. People who eat neither run lower stores.
A trial at Swansea gave 128 young adult women 20g a day of creatine or placebo for five days, double-blind, split by whether they were vegetarian. Creatine did not influence verbal fluency or vigilance in anyone. It produced better memory in the vegetarians and not in the meat-eaters. Regardless of diet, it reduced the variability of responses on a choice reaction-time task — steadier, rather than faster.
Same supplement, same dose, same tests, and the effect landed only where there was a gap to fill.
Sleep deprivation, where the effect is largest
The other strain that produces a signal is having been awake too long. A 2026 randomised trial kept 29 healthy subjects awake for 21 hours and gave them a single dose of creatine monohydrate at 0.2g per kilogram of body weight, or placebo, with cognitive testing at 3, 5.5 and 7.5 hours.
Creatine reduced the deterioration in logical and numerical tasks, in language-related processing speed, and on the psychomotor vigilance test — the standard measure of attention lapses. The reported improvement runs to up to 12%, less than the same group had previously found with a higher single dose of 0.35g per kilogram.
Read the dose before you get excited. For an 80kg adult, 0.2g per kilogram is 16 grams in one sitting — three times a normal daily serving, taken at once, on a night without sleep. This is a laboratory manoeuvre, not a maintenance protocol, and what it produced was less deterioration, not enhancement. The sleep debt itself remains the biggest lever on that curve by a wide margin.
The regulator's read, which we are not going to hide
In 2024 the European Food Safety Authority assessed a formal health claim for creatine and improvement in cognitive function. The panel considered 21 human intervention studies plus two more, and concluded that a cause-and-effect relationship has not been established.
Its reasoning is specific and worth knowing. The acute working-memory effect seen at 20g a day for five to seven days did not appear at 2.2 to 14g a day, nor with 5g a day taken continuously for six weeks after a loading phase. An effect on response inhibition at 20g a day for seven days was an isolated finding among ten trials in healthy people, with nothing in other cognitive domains. And the panel judged the mechanistic evidence weak.
That verdict and the meta-analysis are not contradictory. A regulator asks whether a claim can be made about the general population. A meta-analysis asks whether an average effect exists across pooled studies. You can honestly answer yes to the second and no to the first, and both are useful to a person deciding what to buy.
What we'd actually tell you
If you already take creatine for training, you do not need to change anything, and there is a reasonable chance you are getting a small cognitive side benefit if you are older, vegetarian, or chronically short of sleep. If you are a well-fed, well-rested 25-year-old buying it purely for your brain, the pooled effect in your age band is 0.03 and the regulator says the claim is not established. Buy it for your training instead, where the evidence is not remotely in doubt.
Do not use the sleep-deprivation trials as permission. Sixteen grams of creatine recovering 12% of what an all-nighter took away is not a good trade, and it is a worse trade than going to bed. If you are stacking this with something for the same purpose, note that omega-3 and cognition runs on the same conditional logic — it moves in people who are short, and not in people who are not.

Should I take creatine instead of fixing my sleep?
No, and the trial that most tempts people to is the reason. Sixteen grams of creatine recovering a fraction of what one sleepless night removes is a bad trade against simply sleeping. Chronic short sleep produces attention deficits far larger than anything creatine has been shown to return.
